MI Health Link Waiver Program Overview
The MI Health Link program gives you a range of health care and services in one package. If you qualify for the program, you can get medical and behavioral care, home and community-based services, medication and transportation. You can also receive nursing home care. Plus, you’ll have help from a care coordinator who will help you create a plan and connect you to the services and supports that are important to you.
Who the MI Health Link Waiver Program Serves
- Adults aged 21 years or older
- Michigan residents of certain counties
- Those enrolled in both Medicaid and Medicare
- Adults not living in a state-operated veteran’s home
- Those not currently enrolled in hospice
With the MI Health Link Waiver Program, You [Participants] Can…

- Use one plan and one card for the care you need
- Have a care coordinator who can help you manage everything you need
- Avoid co-pays and deductibles on medication covered under the plan
