Physical Disability HCBS Waiver Program Overview
If you have a physical disability, you can get the support you need with the Physical Disability (PD) waiver–right in your own home. The program serves people with physical disabilities who are between the ages of 16 and 64. The goal of the program is to help you control your own services and supports. You’ll also be able to stay in your own home and community and hire people you trust to provide your services.
To be eligible for the program, you must need a similar level of care to be accepted into a nursing facility (due to a physical disability. You can also be check with the Social Security Administration (SSA). If the SSA determines that you’re disabled eligible for Medicaid, you may also get accepted.
Who the Physical Disability HCBS Program Serves

- Be determined disabled by the Social Security Administration
- Need assistance to perform daily activities
- Meet the Medicaid nursing facility threshold score
- Be financially eligible for Medicaid
