With the Medically Fragile Waiver, families can get support to help care for a child with complex medical needs at home. This program provides services and resources that help children stay with their families and continue living in their community. Support may include in-home care, medical equipment and supplies, transportation, counseling, and respite services. Talk to your case manager. Tell them that you’re interested in Self-Direction.
If you’re eligible, your case manager will make the referral to GT Independence.
