Home and Community Based Waiver Overview
Wisconsin Home and Community Based Services Waiver Programs are Medicaid funded programs which assist individuals to live in their homes or in their community instead of a long-term care facility or nursing home. Medicaid pays for identified services and supports depending on the community waiver program which the individual with a disability has chosen to participate in. The community waiver programs in Wisconsin are:
- Family Care
- Family Care PACE and Partnership
- IRIS (Include, Respect, I Self-Direct)
Who the Home and Community Based Waiver Serves
Criteria which will determine your eligibility in a WI Home and Community Based Waiver Program:
You (are):
- Elderly, blind, or disabled
- Age 18 or older
- Meet the income and asset rules
- Meet the nursing home or non-nursing home functional level of care
- A citizen of qualifying immigrant and a resident of Wisconsin
- For IRIS, you live in your own home or in the community
NOTE: You may also be able to enroll in a community waiver program if you are enrolled in Medicaid or receive SSI (Supplemental Security Income).

GT Independence is a Fiscal Employer Agent (FEA) serving and supporting the State of Wisconsin Family Care, Family Care PACE and Partnership, and the IRIS program. Home and Community Based Waivers are offered through long-term supports and services programs:
- Family Care: Managed Care Organizations
- Family Care PACE and Partnership: Select Managed Care Organizations
- IRIS: IRIS Consultant Agencies
