Oregon Agency With Choice Overview
Oregon Agency with Choice, a GT Independence company, gives older adults, people with disabilities, and people with mental health conditions more support in managing their in-home care services. Through this program, you can choose who provides your care and help train them to meet your needs.
You share employer responsibilities with a partner agency. While you choose and direct your caregiver, the agency handles tasks such as hiring, background checks, payroll, and required paperwork.
This program helps you stay in control of your services while receiving extra support with employer-related responsibilities. Agency with Choice services are available to eligible people through the Office of Aging and People with Disabilities ( APD) and the Oregon Health Authority (OHA).
Which agency supports your services?
The rules and steps are different for APD and OHA. Choose the option below that best describes you.
Older adults and people with physical disabilities (APD)
You may qualify if you:
- Are age 60 or older, or age 18 or older with a physical disability.
- Require help with regular tasks, such as walking, bathing, dressing, or eating.
- Are eligible for services through Aging and People with Disabilities or an Area Agency on Aging.
Individuals with mental health conditions (OHA)
You may qualify if you:
- Are of any age and have a mental health condition.
- Require help with regular tasks, such as bathing, dressing, or eating.
- Have these services included in your approved service plan.
