Personal Choices Program (PCP)

The Personal Choices Program is a program that provides self-directed services at home and in the community. It helps older adults and people with disabilities have more choice and control over the care they receive. People in the program can use their budget to pay for personal care services they need. They can also save money for items that help them stay healthy, safe, and independent.

Three Steps to Get Started

  1. Talk to your case manager. Tell them that you’re interested in Self-Direction.

  2. If you’re eligible, your case manager will make the referral to GT Independence.

  3. Begin self-directing your services with GT Independence.
