How do I get services that are right for me or my family?

With self-determination, you have the right to live the life you choose—right in your own home and community, no matter your age or ability.

Everyone’s situation is different. For some, self-determination means the ability to receive services and support from a loved one. For others, it’s supporting parents so that they can care for children with disabilities. Or, it may simply be that as you get older you want to continue living where you choose and get care from someone you trust. No matter your situation, self-determination helps you choose the services and supports that are right for you.

People choose self-determination for their care for many reasons. GT Independence is here to support your decision. You make all the important choices—like who to hire, when to schedule support, how to manage your care, and even what to spend your budget on. We take care of the paperwork and are here to answer questions you may have.

GT Independence works with these programs in Minnesota:

  • Consumer-Directed Community Supports (CDCS)
  • Consumer Support Grant (CSG)
  • Personal Care Assistance (PCA) Choice / Community First Services and Supports (CFSS)

You deserve to live the life you choose. GT is here to make that easier.

How GT Independence Can Help You

GT Independence is a financial management service (FMS) company that helps you live a life of your choosing. You decide what kind of services and supports you want; we handle the details. For example, you hire employees (also known as providers or caregivers) you know and trust. We assist you by taking care of all the necessary paperwork, such as timesheets, paychecks, taxes, and insurance.

At GT, we believe everyone should be able to decide where to live and who provides their care. Through self-determination, they can. Each person has a unique situation and personalized needs. GT is here to help you learn about how self-determination works in Minnesota, so you have the information to decide what is best for you.

Why Choose GT?

  • We’ve Been In Your Shoes

People with disabilities own and lead GT, and we have focused on self-determination since our start in 2004. We understand that self-determination for your care can feel complicated, but we are here to walk you through the process—from start to support.

  • Self-Determination is All We Do

GT is an FMS company that focuses all our attention on one thing: self-determination. Our people understand how programs work in Minnesota and in states all across the country. We serve thousands of participants nationwide, helping each person live the life of their choosing.

  • Customer Service That Cares About You

Our customer service team is here to help you live life on your terms. Our local Minnesota team is trained on your program. This ensures you always have access to the information you need to make informed decisions. And, we can support more than 200 languages on request.

At GT, we’re ready to assist the person served and their family, as well as their employees. We’re also here for case managers, care coordinators, support planners, and other people involved in services and supports. Our goal is to find frustration-free solutions that fit the lives of the person served and their family.

  • Tools and Resources Designed for the Details

GT has designed many tools to help manage the details of self-determination. A secure online portal helps people self-directing services and their families keep track of services and spending. And the Caregiver app by GT lets employees log their hours and check the approval status of their time entries. We take care of the details, so you can focus on living your life.

“With CDCS, I am in charge of my son’s caregiving, with the most authority over his care. We hire our own staff through CDCS and GT Independence because we have the choice and know who is best for our loved one.”

Consumer-Directed Community Supports (CDCS)

Consumer-Directed Community Supports gives the person self-directing their services and their family more choice in care and cost. Through CDCS, you can select the services and supports that fit your needs. And you can decide when to receive them and who will provide them to you. Plus, you have more control over your Medicaid budget.

Consumer Support Grant (CSG)

The Consumer Support Grant program, funded by the state, gives people self-directing their care and their families a different choice from Medicaid home care services. CSG gives people more freedom to choose how they pay for a home health aide, personal care assistance and/or private duty nursing.

Personal Care Assistance (PCA) Choice / Community First Services and Supports (CFSS)

The Personal Care Assistance (PCA) Choice program and the Community First Services and Supports (CFSS) program let you decide who provides your services. Both programs are funded by state dollars. You get a budget and choose how to use it. You can hire employees to help you, and you can even buy items that help you live more independently.

How to Get Started

Take these steps to begin self-directing your services and use GT as your financial management service:

  1. Contact your lead agency/county to ask for a MNChoices assessment.

  2. Once qualified, let your assessor, case manager, or care coordinator know you want to self-direct your services.

  3. Select GT Independence as your FMS provider and call 877.659.4500 to schedule your in-person enrollment meeting.
