How do I get in-home support for myself or my loved one?

With self-direction, you have the right to live the life you choose. Everyone’s situation is different. For some, self-direction means receiving full-time support from someone they trust–right in their home and community. For others, it might mean hiring a friend to drive them to get groceries or community events a few times a week. The goal is for you to choose what type of support you want and need.

Self-direction puts you in the driver’s seat. This comes with some responsibilities, including working with your coordinator of community service (CCS) to develop a plan that meets your needs. You also manage your own budget and make sure your authorized funds are being spent properly. The GT Portal helps make this part simpler with helpful reports about your budget and utilization management.

Vendor Requirements by Waiver Service

  • Assistive Technology

    1. Assistive Technology
    1.1. For provider agencies rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    1.2. Be at least 18 years of age

    1.3. Have required credentials, license or certification in an area related to the specific type of technology

    1.4. Individuals performing assessments for Assistive Technology (except for Speech Generating Devices) must meet following requirements:

    1.4.1. Rehabilitation Engineering and Assistive Technology Society of North America (RESNA) Assistive Technology Practitioner (ATP);

    1.4.2. California State University Northridge (CSUN) Assistive Technology Applications Certificate; or

    1.4.3. Certificate of Clinical Competence in Speech Language Pathology (CCC-SLP).

    1.5. Individuals performing assessments for any Speech Generating Devices must meet the following requirements:

    1.5.1. Need assessment and recommendation must be completed by a licensed Speech Therapist;

    1.5.2. Program and training can be conducted by a RESNA Assistive Technology Practitioner (ATP) or

    1.5.3. California State University North Ridge (CSUN) Assistive Technology Applications Certificate professional.

    1.6. Assistive Technology Specialist/Practitioner must have an acceptable certification from any of the following:

    1.6.1. Rehabilitation Engineering and Assistive Technology Society of North America (RESNA) Assistive Technology Practitioner (ATP);

    1.6.2. California State University Northridge (CSUN) Assistive Technology Applications Certificate; or

    1.6.3. Certificate of Clinical Competence in Speech Language Pathology (CCC-SLP); and d. Minimum of three years of professional experience in adaptive rehabilitation technology in each device and service area certified

    1.7. Licensed professional must have:

    1.7.1. Maryland Board of Audiologists, Hearing Aid Dispensers & Speech-Language Pathologists license for Speech-Language Pathologist, or

    1.7.2. . Maryland Board of Occupational Therapy Practice license for Occupational Therapist.

    1.8. Background checks for all staff members who rendered services

    1.9. Child Protective Services background checks for staff members who rendered services to a participant under the age of 18, unless otherwise authorized by DDA.

    1.10. Have Commercial General Liability Insurance

  • Community Development

    2. Community Development
    2.1. For individuals rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    2.2. Be at least 18 years of age

    2.3. Must possess current in person First Aid and CPR Certification

    2.4. Background checks for all staff members who rendered services

    2.5. Child Protective Services background checks for staff members who rendered services to a participant under the age of 18, unless otherwise authorized by DDA.

    2.6. Unlicensed direct support professional staff who administer medication or perform delegable nursing tasks as part of this Waiver service must be certified by the Maryland Board of Nursing (MBON) as Medication Technicians, except if the participant and his or her medication administration or nursing tasks qualifies for exemption from nursing delegation pursuant to COMAR 10.27.11

    2.7. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    2.8. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services; 8. Complete required orientation and training designated by DDA

    2.9. For provider agencies rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    2.9.1. Complete the DDA provider application and be certified based on compliance with all of the following standards:

    2.9.2. Be properly organized as a Maryland corporation, or, if operating as a foreign corporation, be properly registered to do business in Maryland

    2.9.3. A minimum of five (5) years’ demonstrated experience and capacity providing quality similar services

    2.9.4. Have a governing body that is legally responsible for overseeing the management and operation of all programs conducted by the licensee including ensuring that each aspect of the agency’s programs operates in compliance with all local, State, and federal requirements, applicable laws, and regulations

    2.9.5. Be in good standing with the IRS and Maryland Department of Assessments and Taxation

    2.9.6. Have Workers’ Compensation Insurance

    2.9.7. Have Commercial General Liability Insurance

    2.9.8. Submit results of criminal background checks, Medicaid Exclusion List, and child protective clearances as provided in Appendix C-2-a and as per DDA policy

    2.9.9. Submit documentation of staff certifications, licensees, and/or trainings as required to perform services

    2.10. Staff working for the agency must meet the following minimum standards:

    2.11. Be at least 18 years of age

    2.12. Must possess current in person First Aid and CPR Certification

    2.13. Background checks for all staff members who rendered services

    3.5. Pass a criminal background investigation and any other required background checks and credentials verifications as provided in Appendix C-2

    3.6. Have DDA approved certification in employment to provide discovery services

    3.7. Unlicensed direct support professional staff who administer medication or perform delegable nursing tasks as part of this Waiver service must be certified by the Maryland Board of Nursing (MBON) as Medication Technicians, except if the participant and his or her medication administration or nursing tasks qualifies for exemption from nursing delegation pursuant to COMAR 10.27.11

    3.8. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    3.9. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services; 8. Complete required orientation and training designated by DDA

    3.10. For provider agencies rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    3.11. Complete the DDA provider application and be certified based on compliance with meeting all of the following standards

    3.11.1. Be properly organized as a Maryland corporation, or, if operating as a foreign corporation, be properly registered to do business in Maryland

    3.11.2. A minimum of five (5) years demonstrated experience and capacity providing quality similar services

    3.11.3.     3.19.7. Complete necessary pre/in-service training based on the Person-Centered Plan

    3.19.8. Complete all DDA required training prior to service delivery

    3.19.10. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services.

  • IFDGS Day to Day Admin

    4. IFDGS Day to Day Admin
    4.1. Be at least 18 years of age

    4.2. Possess current first aid and CPR certification

  • Nursing Support Services

    5. Nursing Support Services
    5.1. For individuals rendering services, proof of the following items must be sent prior to rendering services:

    5.2. Possess a valid Maryland and/or Compact Registered Nurse license

    5.3. Successful completion of the DDA RN Case Manager/Delegating Nurse (CM/DN) training within 90 days of first providing services Orientation

    5.4. Once completed DDA’s training, maintain active status on DDA’s registry of DD RN CM/DNs Be active on the DDA registry of DD RN CM/DNs

    5.5. Complete the online HRST Rater and Reviewer training

    5.6. Attend mandatory DDA trainings

    5.7. Attend a minimum of two (2) DDA provided nurse quarterly meetings per fiscal year

    5.8. Pass a criminal background investigation and any other required background checks and credentials verifications as provided in Appendix C-2-a

    5.9. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    5.10. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services

    5.11. Have Commercial General Liability Insurance

    5.12. Complete necessary pre/in-service training based on the Person-Centered Plan and DDA required training prior to service delivery

    5.13. Have three (3) professional references which attest to the provider’s ability to deliver the support/service in compliance with the Department’s values in Annotated Code of Maryland, Health General, Title 7

    5.14. Demonstrate financial integrity through IRS, Department, and Medicaid Exclusion List checks

    5.15. Have a signed Medicaid Provider Agreement

    5.16. For provider agencies rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    5.17. Complete the DDA provider application and be certified based on compliance with meeting all of the standards as outlined in Appendix C.

    5.18. Be in good standing with the IRS and Maryland Department of Assessments and Taxation

    5.19. Have Workers’ Compensation Insurance

    6.6. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

Unlicensed direct support professional staff who administer medication or perform delegable nursing tasks as part of this Waiver service must be certified by the Maryland Board of Nursing (MBON) as Medication Technicians, except if the participant and his or her medication administration or nursing tasks qualifies for exemption from nursing delegation pursuant to COMAR 10.27.11

    7.6. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    7.7. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services

    7.8. Have three (3) professional references which attest to the provider’s ability to deliver the support/service in compliance with the Department’s values in Annotated Code of Maryland, Health General, Title 7

    7.24. Pass a criminal background investigation and any other required background checks and credentials verifications as provided in Appendix C2-a

    7.25. Unlicensed direct support professional staff who administer medication or perform delegable nursing tasks as part of this Waiver service must be certified by the Maryland Board of Nursing (MBON) as Medication Technicians, except if the participant and his or her medication administration or nursing tasks qualifies for exemption from nursing delegation pursuant to COMAR 10.27.11

    7.26. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    8.2. Be at least 18 years old

    8.3. Current first aid and CPR certification

    8.4. Pass a criminal background investigation and any other required background checks and credentials verifications as provided in Appendix C-2-a

    8.5. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    8.6. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services

    8.7. For agencies’ rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    8.8. Complete the DDA provider application and be certified based on compliance with meeting all of the standards as outlined in Appendix C.

    8.9. Be in good standing with the IRS and Maryland Department of Assessments and Taxation

    8.10. Have Workers’ Compensation Insurance

    8.17. Be certified by the DDA to demonstrate core competency related to self-determination, consumer directed services and service systems (generic and government-sponsored) for individuals with disabilities and effective staff management strategies

    8.18. Complete required orientation and training designated by DDA including the Policy on Reportable Incidents and Investigations (PORII) and Support Broker trainings

    8.19. Complete necessary pre/in-service training based on person-specific information (including preferences, positive behavior supports, when needed, and disability-specific information as noted in the Person Centered Plan and DDA required training prior to service delivery

    8.20. Possess current first aid and CPR certification

    8.21. Pass a criminal background investigation and any other required background checks and credentials verifications as provided in Appendix C-2-a

    8.22. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    8.23. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services

  • Transportation

    9. Transportation
    9.1. For individuals or vendors rendering services, proof of the following items must be sent prior to rendering services:

    9.2. Be at least 18 years old

    9.3. Have required credentials, license, or certification as noted below as applicable

    9.4. Pass a criminal background investigation and any other required background checks and credentials verifications as provided in Appendix C-2-a for noncommercial drivers

    9.5. Possess a valid driver’s license for non-commercial drivers

    9.6. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of service for noncommercial providers

    9.7. Have three (3) professional references which attest to the provider’s ability to deliver the support/service in compliance with the Department’s values in Annotated Code of Maryland, Health General, Title 7

    9.8. Demonstrate financial integrity through IRS, Department, and Medicaid Exclusion List checks

    9.9. Have a signed Medicaid Provider Agreement

  • Shared Living

    10. Shared Living
    10.1. For individuals rendering services, proof of the following items must be sent prior to rendering services:

    10.2. Be at least 18 years old

    10.3. Have a GED or high school diploma unless previously approved by the DDA

    10.4. Possess current First Aid and CPR training and certification

    10.5. Pass a criminal background investigation and any other required background checks and credentials verifications as provided in Appendix C-2-a

    10.6. For agencies’ rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    10.7. Complete the DDA provider application and be certified based on compliance with meeting all of the standards as outlined in Appendix C.

    10.8. Be in good standing with the IRS and Maryland Department of Assessments and Taxation

    11.3. Have a Bachelor’s Degree, professional license, certification by a nationally recognized program, or demonstrated life experiences and skills to provide the service

    11.4. Possess a valid driver’s license, if the operation of a vehicle is necessary to provide services

    11.5. Have documentation that all vehicles used in the provision of services have automobile insurance

    11.6. Have three (3) professional references which attest to the provider’s ability to deliver the support/service in compliance with the Department’s values in Annotated Code of Maryland, Health General, Title 7

    11.7. Demonstrate financial integrity through IRS, Department, and Medicaid Exclusion List checks

    11.8. Have a signed Medicaid Provider Agreement

    11.9. For provider agencies rendering services, proof of the following items must be sent to GT prior to the individual rendering services:

    11.10.1. This includes all requirements and standards as listed in Appendix C

    11.11. Be in good standing with the IRS and Maryland Department of Assessments and Taxation

    11.12. Have Workers’ Compensation Insurance

    11.14. Submit results from required criminal background checks, Medicaid Exclusion List, and child protective clearances as provided in Appendix C-2-a and per DDA policy

    16.6. Have automobile insurance for all automobiles that are owned, leased, and/or hired and used in the provision of services

    20. Vehicle Modifications
    20.1. Be at least 18 years of age

    20.2. Be a Division of Rehabilitation Services (DORS) Vehicle Modification service vendor

    20.3. Vehicle Equipment and Adaptation Prescription Agreement (VEAPA) must be completed by a driver rehabilitation specialist or certified driver rehabilitation specialist

    20.4. The adaptive driving assessment specialist who wrote the Adapted Driving Assessment report and the VEAPA shall ensure the vehicle modification fits the consumer and the consumer is able to safely drive the vehicle with the new adaptations/equipment by conducting an on-site assessment and provide a statement as to whether it meets the individual’s needs

Community Personal Assistance Services

How to Get Started

Take these steps to begin self-direction with GT Independence.

  1. Talk with your coordinator of community services (CCS) or support broker. Tell them that you’re interested in self-direction.

  2. If you’re eligible, your CCS will make a referral through LTSS Marlyand. Make sure to tell your CCS that you want GT Independence as your financial management services and counseling provider (FMCS).

  3. Begin self-directing your services with GT Independence.
