How do I get services that are right for me or my family?
With self-determination, you have the right to live the life you choose—right in your own home and community, no matter your age or ability.
Everyone’s situation is different. For some, self-determination means the ability to receive services and support from a loved one. For others, it’s supporting parents so that they can care for children with disabilities. Or, it may simply be that as you get older you want to continue living where you choose and get care from someone you trust. No matter your situation, self-determination helps you choose the services and supports that are right for you.
People choose self-determination for their care for many reasons. GT Independence is here to support your decision. You make all the important choices—like who to hire, when to schedule support, how to manage your care, and even what to spend your budget on. We take care of the paperwork and are here to answer questions you may have.

Consumer-Directed Community Supports (CDCS)
Consumer-Directed Community Supports gives the person self-directing their services and their family more choice in care and cost. Through CDCS, you can select the services and supports that fit your needs. And you can decide when to receive them and who will provide them to you. Plus, you have more control over your Medicaid budget.

Consumer Support Grant (CSG)
The Consumer Support Grant program, funded by the state, gives people self-directing their care and their families a different choice from Medicaid home care services. CSG gives people more freedom to choose how they pay for a home health aide, personal care assistance and/or private duty nursing.

Personal Care Assistance (PCA) Choice / Community First Services and Supports (CFSS)
The Personal Care Assistance (PCA) Choice program and the Community First Services and Supports (CFSS) program let you decide who provides your services. Both programs are funded by state dollars. You get a budget and choose how to use it. You can hire employees to help you, and you can even buy items that help you live more independently.
How to Get Started
Take these steps to begin self-directing your services and use GT as your financial management service:

Contact your lead agency/county to ask for a MNChoices assessment.

Once qualified, let your assessor, case manager, or care coordinator know you want to self-direct your services.

Select GT Independence as your FMS provider and call 877.659.4500 to schedule your in-person enrollment meeting.
