How do I hire long-term services for myself or a loved one?
With self-direction, you have the right to live the life you choose—right in your own home and community, no matter your age or ability.
Everyone’s situation is different. For some, self-direction means the ability to receive services from a loved one. For others, it’s supporting parents so that they can care for children with disabilities. Or, it may simply be that as you get older, you want to continue living where you choose and get care from someone you trust. No matter your situation, self-direction helps you hire the long-term care and support that’s right for you.
People choose to self-direct their care for many reasons. GT Independence is here to support your decision. You make all the important choices—like who to hire, when to schedule services, how to manage your care, and even who to fire if things aren’t working out. We take care of the paperwork and are here to answer questions you may have.

MI Choice Waiver Program
The MI Choice Waiver Program is for adults who are eligible for Medicaid. You may be able to receive skilled nursing services in your own home or other setting. The program also provides community living supports, homemaking services, personal care, and non-medical transportation.

Community Mental Health Waiver Programs (CMH)
Community Mental Health agencies work with many different programs. We partner with CMHs to support all the programs and waivers they serve.
Children Waiver Program
The Children’s Waiver Program (CWP) helps with supports and services for children at home. The program is for those who need medical or behavioral attention on a daily basis.
MI Health Link Waiver Program
MI Health Link offers many services in a single program. Medical and behavioral health services, pharmacy, home and community-based services, and nursing home care are all available.
Program for All Inclusive Care for the Elderly (PACE)
The Programs of All-Inclusive Care for the Elderly (PACE) is for people want to continue living where in their own homes and communities. The program offers medical and social services through teams of health professionals. People who choose PACE can self-direct specific services. These services are often eligible for both Medicare and Medicaid.
Veteran-Directed Care
Through the Veteran Directed Care (VDC) program, you can self-direct your long-term service and supports. If you are a veteran, you may be able to keep living independently and at home. And, you could be in control of using your flexible budget to meet your needs.
Three Steps to Get Started

Find your local Area Agency on Aging (AAA) or Community Mental Health (CMH). You can also ask your health plan about self-direction.

Find a program that fits your needs.

Choose GT Independence as your financial management service (FMS) and begin self-directing your services.
